By participating in any Master The Game (MTG) activity — including but not limited to training sessions, camps, leagues, events, or related programs — I acknowledge that I have read this Waiver & Participation Agreement and agree to its terms on my own behalf (if 18 or older) or on behalf of the minor participant(s) for whom I am the parent/guardian or authorized representative.

Participation in any MTG activity constitutes acceptance of this agreement and is required for involvement in any Master The Game programs.

In consideration of being permitted to participate in any Master The Game (MTG) programs, sessions, camps, leagues, training, events, or related activities (the “Activities”), I agree as follows:

1. Acknowledgment of Risk: I acknowledge that participation in soccer and related activities involves inherent risks, including but not limited to physical injury (minor or serious), that cannot be eliminated regardless of the care taken to avoid injuries.
2. Assumption of Risk: I assume all risks, known and unknown, related to the participant’s involvement in the Activities, including travel to and from activities, field conditions, weather, equipment, other participants, and play.
3. Medical Treatment: I authorize the seeking, obtaining, or approval of any routine, necessary, or emergency health care during the participant’s involvement in the Activities, if necessary. I understand that this authorization covers all required diagnosis, treatment, or medical care and serves as advance consent for such actions. I accept full responsibility for any related costs.
4. No Claims Agreement / Release of Liability: I agree that neither the participant nor I will make a claim against, sue, or attach the property of the Coaches, employees, volunteers, MTG – Master The Game, program partners, facility owners, or related parties for any injury or damage resulting from the participant’s involvement in the Activities.
5. Photographic Release: I grant permission to Master The Game (MTG) to use photographs or video recordings of the participant taken during the Activities for promotional purposes only, including but not limited to social media, websites, and other promotional materials.
6. Parental Consent: I certify that I am the parent or legal guardian of the participant(s) (or the participant if 18+) and have the authority to make this agreement. I acknowledge that I have read and fully understand the terms and conditions of this waiver and release.

By participating in any Master The Game (MTG) activity,
I confirm the above statements, have read and fully understand all terms,
understand that substantial legal rights are being waived,
and agree to them freely and voluntarily on behalf of the participant(s).

By submitting this registration, you confirm that you have read, understood, and agree to our Waiver & Participation Agreement.
